To request a repeat prescription
- Use the Evergreen Life app
- Use the Evergreen Life website (Login or Signup)
- Use the Patient Access service
- Complete the right-hand section of your previous prescription form. Leave it with the surgery or send it to us by post
Repeat prescriptions will be ready for collection 72 working hours after ordering.
You can ask us to arrange for your prescription to be collected by the chemist of your choice. If you request this service, your prescription will be ready for collection from the chemist 48 hours later to allow them time to make up your medication.
Patients who receive regular prescriptions will sometimes be asked to see a doctor or practice nurse before their next repeat prescription is due. This is so that your medication can be assessed and altered as necessary.
Your doctor or nurse will try to find the best way of making you better. You may not always be given medicine, as there are many other ways to treat you. Please do not expect a prescription on every visit.
NHS prescription charges
Most adults in England have to pay prescription charges but some people can get free prescriptions.
Need a little help (with the Evergreen App)?
To help you get started with the Evergreen app and creating your account there are a series of help articles available. Linking your account to your GP record will enable the full functionality of the app including records access and repeat prescriptions as well as enabling you to make requests for help online.
We’ve also created a walk-through video to show the steps of using the app for the first time.
The Evergreen team can help you with technical issues, or reset linkage codes if you used these in the past with other services. Please note the Evergreen team cannot assist you with medical queries, nor can they verify identify documents if you have not been registered with us for online services before.
